   Pick the coverage gap you fill

   Teleradiology sells into specific gaps: overnight and weekend coverage (historically called nighthawk), subspecialty reads a rural hospital cannot staff, and overflow capacity during volume spikes. Decide which gap you serve, because it drives your radiologist roster, your turnaround-time promise, and your pricing.    Solve multi-state licensing and credentialing early

   A radiologist must hold a medical license in the state where the patient is located and be credentialed at each client hospital. Serving facilities across many states means many licenses and many credentialing files per radiologist, which is the real barrier and the real moat.    You need a HIPAA-compliant pipeline that receives images (DICOM) from client PACS, presents them on a diagnostic-quality worklist, and returns structured reports, with encryption, audit trails, and reliable uptime. Buy or license a teleradiology worklist and viewer rather than build one at first. Turnaround time and reliability are what clients measure, so the platform is core, not overhead.

   Contract malpractice that covers remote reads

   Malpractice coverage must span every state you read into and the remote-interpretation model specifically. This is more complex and often more expensive than single-site coverage, and it is non-negotiable. Confirm coverage before your first read, not after.

   Win and keep hospital and imaging-center contracts

   Sell to hospitals, imaging centers, and hospital-based radiology groups that need coverage. Compete on turnaround time, subspecialty depth, report quality, and reliability, and price per study or as a monthly coverage retainer. Contracts renew on service metrics, so track turnaround, discrepancy rates, and critical-result communication from day one.

   A small solo remote-reading service (sometimes bundled with pathology) is a lighter, lower-capital version already in this library.
