Which markup column should I quote from?
Most local staffing lands between 45% and 65%; specialized clinical and hard-to-fill roles justify 65% to 75%. The right column is the one that covers your burden with room left; the Burden Builder tells you where that line is.
How is each bill rate calculated?
Bill = pay rate x (1 + markup). The $20 row reads $29.00 at 45%, $31.00 at 55%, $33.00 at 65%, and $35.00 at 75%; identical math to the Markup calculator, precomputed for speed.
Does this include burden or profit?
No, it is a quoting reference: markup over pay only. Your real profit is the spread minus burden. Quote from the sheet, then run the Markup calculator with your burden before you sign anything.
