   This industry's reputation problem is no-shows and late pickups for people who cannot afford to miss dialysis.    Run scheduling as the profit engine

   Recurring trips (dialysis three times weekly, day programs daily) build routable density, and route optimization software turns scattered pickups into profitable loops. Know your per-trip economics cold: broker rates vary by state and trip type, deadhead miles kill margin, and the operators who thrive are the ones whose vans are rarely empty.

   Climb toward ambulance deliberately

   Licensed ambulance service (basic life support transfers, then emergency response) requires state EMS agency licensure, certified EMTs and paramedics, medical direction, and far heavier vehicles and insurance; interfacility transfer contracts with hospitals are the usual entry. It is a genuine step up in complexity and capital; take it only after NEMT operations run themselves, and veterans with military medic backgrounds hold a real head start on the staffing side.
