   Medicaid rates run leaner than commercial rates, so the business must be built for its actual reimbursement: efficient scheduling, tight documentation, clean claims, and services chosen because their rates support the model.

   States expand waiver programs as they push care into homes and communities. Following your state's waiver expansions (and adding neighboring states once one operation runs clean) is the scaling path.
