   Lease a space that meets accessibility, safety, and space-per-child standards with sensory-considered treatment areas. Then hire registered behavior technicians (RBTs) who deliver direct therapy under BCBA supervision, keeping supervision ratios sound. RBT recruitment, training, and retention are the operational core of a center; turnover is the field's biggest risk, so pay fairly and design caseloads that prevent burnout.

   Run intake, authorization, and scheduling tightly

   A center's economics depend on filling authorized hours: intake, assessment, prior authorization, and daily scheduling must run cleanly so authorized children are actually seen for their approved hours. Track utilization, cancellations, and re-authorizations weekly. In a focused clinic these systems are simpler than in an agency, which is part of the model's appeal, but they are still what determines whether you get paid for the care you deliver.

   Grow through referrals and clinical reputation

   Pediatricians, developmental specialists, early-intervention programs, and families are the referral engine. A focused center earns referrals by being reliable, affirming, and outcomes-driven, not by advertising. Grow caseload and add BCBA and RBT capacity in step with demand and authorization, rather than overbuilding ahead of billable volume.
