   Antepartum clients are pregnant people on bed rest, managing conditions like preeclampsia risk or preterm labor threats, or simply navigating a pregnancy their medical team has labeled complex. Your work is emotional support, comfort measures, companionship through long anxious days, light household help, sibling care coordination, and helping the client prepare questions for medical appointments. You never interpret test results, monitor vitals, or give medical opinions; the clinical team does medicine, you do everything that makes the waiting bearable.

   Train through the established doula bodies

   Most major certifying organizations treat antepartum support as an extension module on top of birth doula training rather than a separate credential, so start with a recognized program. Expect 3 to 12 months to complete certification requirements depending on how quickly you log the attendance and evaluation pieces.

   Maternal-fetal medicine practices, OB offices with high-risk panels, midwives who risk clients out to specialist care, and hospital antepartum units all meet your exact client on the hardest day of her pregnancy. Perinatal mental health therapists and childbirth educators are the second ring. One or two clinicians who trust your boundaries can keep a solo practice full.

   Bundle forward into birth and postpartum

   An antepartum client who trusted you through eight weeks of bed rest rarely wants a stranger at the birth. Offer continuation packages into labor support and postpartum shifts, or partner with birth and postpartum doulas you refer into if you keep this stage as your specialty. Either way, the antepartum relationship is the deepest client bond in the doula field; build your business model around its natural continuations.
